In order to register with the practice, please complete the forms below. 

You will need to come into the surgery with these forms and your passport or birth certificate and proof of your address (please note that driving licences cannot be accepted as proof of address).

Please contact your registered practice as they may be able to help you over the phone or send medications to one of the local pharmacies. You can also call 111 who can direct you to alternative medical services. 111 are also able to book an appointment with the surgery. Please try these services first.

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

All disabled and partially sighted patients should access the Princes Risborough Surgery through the rear entrance to avoid any steps.

If you require any assistance in accessing the building or its services, please telephone reception from outside on your arrival.

When booking an appointment, please inform the receptionist that you will require a room with disabled access so that the right room can be allocated to you.
